This is the administrative office that coordinates the services of several public health department divisions: Center for Chronic Disease Prevention & Health Promotion; Center for Infectious Disease; Center for Environmental Health; Center for Healthcare Quality and Food and Drug Branch. Services include statewide Public Health Advisories, public health complaints, vital records and west nile virus control. See the separate program entries for detailed service descriptions. There are no geographic restrictions.
The Office of Vital Records can mail copies of birth certificates, death certificates, marriage certificates, and divorce decrees. The process is lengthy, and the department recommends that people obtain these documents through the County Recorders Office in the County in which the event occurred; however, where time is not an issue, the records are available from this office by mail.
For the West Nile Virus the agency has partnered with UC Davis Center for Vectorborne Diseases, Mosquito and Vector Control Association of California and the California Department of Food and Agriculture to provide a website with information about the West Nile Virus in California. The California West Nile Virus web site is at www.westnile.ca.gov and individuals can also submit an online Dead Bird report or call the hotline. APPLICATION PROCEDURE Call, write or visit the department's website for information or to request records. Call the Dead Bird Hotline or visit the California West Nile Virus website to report a dead bird. FEES/PAYMENT SOURCE There are fixed fees for some public health services; see the separate program entries.
