The program provides end of life services targeted to individuals with a confirmed diagnosis of a terminal illness and a limited life expectancy living in the Antelope Valley. Services include bereavement counseling, bereavement support groups, and hospice care. There are no geographic restrictions.

Hospice services include pain control and symptom management, support and help for patients and family members to deal with fear, loneliness, anxiety, worry and feelings of helplessness.

Services enables a terminally ill person to decide how and where the rest of his or her life should be lived. Hospice care or palliative care focuses on comfort and quality of life.

The program provides skilled nursing, home health aide care, medical social work, pastoral counseling, bereavement counseling, volunteer services, medical equipment, medical supplies, medications related to hospice diagnosis and bereavement services and counseling for the family and loved ones for up to one year after their loss. APPLICATION PROCEDURE Call for information. A physician referral is required for individuals to receive service. FEES/PAYMENT SOURCE The agency collects fees from private insurance, Medi-Cal, Medicare, Champus and HMOs.

Office hours are Monday through Friday, 8:00am to 5:00pm; 24 hour per day on-call answering service for after-hours call nurse is available at all other times.
