This agency provides comprehensive outpatient health services for people of all ages who live primarily in the South Central Los Angeles area from five locations. Services are targeted, but not restricted, to the African-American and Spanish-speaking communities of South Central Los Angeles. Services include blood pressure screening, child health and disability prevention (CHDP) examinations, childhood immunizations, community clinics, contraception, contract clinics, dental care, diabetes screening, disease/disability information, family planning, general medical care, immunizations, mobile health care, obstetrics/gynecology, ophthalmology, pediatrics, pharmacies, physical therapy, podiatry/foot care, prenatal care, AIDS drug assistance programs, sexuality/reproductive health education, smoking cessation, stroke rehabilitation and urgent care centers. There are no geographic restrictions; income and other restrictions apply for some programs.

CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.

Case management includes assistance with psychological needs, housing, food, medical needs and community referrals.

Clinic services include dental, family planning, gynecology, immunization, ophthalmology, pediatrics, physical therapy and stroke rehabilitation. Medical care is provided to homeless people through mobile medical units. Ancillary services include a lab, radiology unit, pharmacy and transportation to and from the center.

Dental services for adults and children include examinations, cleaning, fillings, fluoride treatments, x-rays, extractions, and some root canals.

Health education programs include health screening and education about diabetes and hypertension. Education is provided about AIDS, family planning, nutrition, parenting, smoking cessation, substance abuse and weight control. Information is available via literature, seminars, health fairs and a speakers bureau.

The center enrolls its own patients for the AIDS Drug Assistance Program (ADAP), which provides FDA-approved HIV-related drugs at little or no cost to those who are eligible; call for more information.

APPLICATION PROCEDURE Call for additional information or an appointment. An in-home intake interview is required for the geriatrics and homebound program. Dental clients must show proof of income and address. People may walk in for dental emergencies. FEES/PAYMENT SOURCE Clinic fees vary according to services and are set primarily on an ability to pay basis or through the foundation's pre-paid health plan. Medi-Cal and private insurance are accepted. Dental fees start at $25 per visit and depend on the type of service.

Monday through Wednesday, 8:00am to 6:00pm; Thursday and Friday, 8:00am to 5:00pm; the second and last Saturday of the month, 7:30am to 4:00pm. The urgent care clinic hours are Monday through Saturday, 10:00am to 7:00pm. Pharmacy hours are Monday through Friday, 9:00am to 8:00pm; Saturday, 9:00am to 5:30pm.
