Case/Care Management ( Frail Elderly )

Provider: WATTS LABOR COMMUNITY ACTION COMMITTEE - BRADLEY MULTIPURPOSE SENIOR CITIZEN CENTER

The center provides a variety of services for older adults age 60 and older who live in the 8th and in the upper portion of the 15th Council Districts of South Central Los Angeles. Services include case/care management, certificate/forms assistance, friendly visiting, recreational activities/sports, senior centers, specialized information and referral and telephone reassurance.

Services also provided include educational workshops; crime education; recreational and social activities; nutrition education; and a WLCAC newspaper which informs people about dates and times of specific workshops, classes, services, and activities.

The center provides case management services for people who can benefit from assistance in evaluating, coordinating, and monitoring the services they need. The Supportive Services Program (SSP) provides case management services to assess the physical, psychological, and social needs of older adults age 60 and older to allow more frail seniors to stay in their homes as long as appropriately possible.

The In-Home Services program provides volunteer visitors for people in the area who are homebound and unable to attend center activities. In addition, people may request weekly phone calls from center volunteers in the telephone reassurance program. Other services include legal assistance. The center also offers numerous recreational and social activities and classes including bingo, dominos, arts and crafts, consumer education, and educational and recreational trips. APPLICATION PROCEDURES Call for information about any of the center's services. FEES/PAYMENT SOURCE There are no fees for some services; however, donations are suggested. No one is refused services due to inability to pay.
