Case/Care Management ( Youth, Families/Friends of People With Disabilities )


The agency provides services for people with developmental disabilities, age 13 and older and their families. Services include independent living skills instruction and supportive therapies; and case management for youth ages 7 to 17 living in households with developmentally disabled individuals. Age restrictions apply; there are no geographic restrictions.

Living skills instruction and supportive therapies are provided for adults who are referred through the Regional Centers.

The Rise Program provides case management for youth living in homes with developmentally disabled parents, grandparents or other heads of household. Staff work with clients and their families to identify and reduce risk factors and to promote independence and self-empowerment. Supportive services include functional skills assessments, linkages with other social services and mentoring. Workshops on social skills and higher education opportunities are also available. APPLICATION PROCEDURE Call to apply; proof of verified disability is required. FEES/PAYMENT SOURCE There are no fees for services; some fees are paid directly through the Regional Centers.
