Case/Care Management ( Homeless Youth )

The drop in center provides a safe place where youth can access a range of supportive services, improve physical and mental health, and experience a caring connection. Services may include housing referrals; return home travel; hot meals; clothing; blankets; hygiene products; first aid kits; sleeping bags; bus fare; internet and phone access; mail; onsite medical clinic that offers care for basic wounds and infections as well as referrals, HIV and Hepatitis C testing; harm reduction interventions; health education workshops; legal assistance; dental care; pet care; creative expression classes, including yoga; drum circles, creative writing and a Digital Arts Lab with 3D printers.
Street outreach teams engage one-on-one with youth at-risk for or who are experiencing homelessness in Venice; and build relationships while providing street-based case management and survival services. Services provided include snacks, hygiene packs, socks, HIV/STD safety kits as well as linkages to emergency shelter, mental health and physical care, showers, lockers and on-site services.
Case/care management is provided two days a week and is by appointment. The team works with youth to develop individualized plans to address short and long term goals toward greater stability. Services include assistance to enter emergency shelter, apply for identification documents and birth certificates, health benefits, school enrollment, employment, parenting and prenatal care, and extended guidance in decisions on stable housing, family reunification, drug rehabilitation and life skills.
