Case/Care Management ( Frail Elderly )

The center provides services for older adults age 60 and older who live in Inglewood. Services include information and referral to resources which assist seniors, recreational and educational programs, congregate and home-delivered meals, and specialized programs for seniors whose ability to leave home is limited. Geographic restrictions apply.

Information is available about employment, insurance, health care and government benefits; seniors may also receive assistance in completing forms that relate to programs like Medi-Cal or Social Security. Seniors may also receive education and guidance about Medicare-related issues through the HICAP (Health Insurance Counseling and Advocacy Project; see the separate entry) which sends a staff person to the center every Thursday morning until noon.

A congregate meal program provides a hot lunch at the center five days per week.

Home delivered meals are available for seniors age 60 and older in Inglewood zip codes 90301, 90302, 90303 and 90305; medical verification of need is required. Meals are usually delivered five days per week but Saturday delivery can be arranged in emergency situations.

Telephone reassurance, friendly visiting, and short-term case management assistance may also be available. Trained caseworkers are available to go to seniors' homes, discuss with him or her the supportive services s/he may need, and to make arrangements for them to be provided.

The Supportive Services Program (SSP) provides case management services to assess the physical, psychological, and social needs of older adults age 60 and older to allow more frail seniors to stay in their homes as long as appropriately possible. APPLICATION PROCEDURE Call or walk in for information or for services. People may be referred or may self-refer. People who are interested in the home-delivered meal program may call to apply. When home delivered meals has a waitlist, priority is given on a case-by-case basis. HICAP services are provided by appointment. FEES/PAYMENT SOURCE There are no fees for most center services; however there is a suggested donation of $2.00 per congregate meal and for the home-delivered meals. No one is refused meals due to inability to pay. A donation is suggested for forms completion.
