Case/Care Management ( Pregnant Teens, African American Community )
Provider: BLACK INFANT HEALTH PROGRAM OF ANTELOPE VALLEY
The program provides health education for African American pregnant and parenting women and teens age 16 and older, and their infants up to 18 months, who are in the Antelope Valley area. Services include case management. Services are restricted to residents of zip codes 93534, 93535, 93543, 93550, 93551, 93552 and 93591.
Case management may include information and support about alcohol and drug abuse, child development, preterm labor and low birth weight, prenatal depression, nutritional health, general women's health, prenatal care and breastfeeding training. The program may also assist with smoking cessation, and provide specialized information and referral to health and social services programs.
Referrals are made to prenatal care providers, free or low cost medical care, WIC, childhood immunizations, and other health related services. APPLICATION PROCEDURE Call to apply. FEES/PAYMENT SOURCE There are no fees for services.
