Certificates/Forms Assistance ( Immigrants, Public Assistance Issues )

Provider: LOS ANGELES COUNTY TREASURER AND TAX COLLECTOR - COLLECTION SERVICES

The program provides certificates/forms assistance to immigrants who need to document that they have received or never received public assistance in L A County for INS Public Charge determination purposes. Persons needing to obtain this information for housing programs, the IRS or any other purposes must contact the L A County DPSS Central Helpline (see separate entry) for assistance. There are no geographic restrictions.

The program is authorized exclusively by the INS to provide this documentation for immigrants who currently or no longer live in L A County. The program will provide full report, which includes amounts and dates received; or partial documentation, which only lists type of aid received and neither amounts or dates received. APPLICATION PROCEDURE Call or write for an application. Picture ID with name and address, a list of any previous names or addresses, names and birthdates of children, and his/her mother's and father's name are required. Persons should also provide any forms or letters from the INS requesting documentation of aid received. Allow 10-12 weeks for application to be processed. FEES/PAYMENT SOURCE There are fixed fees, call for more information. Cash or money order are accepted.
