The program provides health education for individuals in Los Angeles County. Services include disease/disability specific screening/diagnosis for heart disease, nutrition education for diabetics. There are no geographic restrictions.

The heart health screening exam with an EKG includes an assessment of the EKG read by a cardiologist, cholesterol screening, gluclose level check, blood pressure check, body mass index (BMI) check and a health risk assessment. This is not available to individuals who have seen a cardiologist or have had an EKG within the last six months.

The nutrition education class for individuals with diabetes is a monthly meeting to learn more about the relationship between good nutrition and diabetes and to develop further knowledge so individuals can make healthier food choices. APPLICATION PROCEDURE Call to register; an appointment is required for the heart health screening. FEES/PAYMENT SOURCE There are no fees for the diabetes nutrition class. There is a $40 fee for the heart health screening; cash, check or credit card only for the screening. Insurance is not accepted for the heart screening.
