Durable Power of Attorney for Health Care ( Developmental Disabilities )

The agency provides services for children age three and older and adults with developmental disabilities in the San Gabriel Valley. Services include bereavement counseling, case/care management, conservatorship assistance, durable power of attorney, independent living skills instruction, individual advocacy, and trust preparation assistance. There are no geographic restrictions.

Independent living skills instruction is provided to help individuals increase or maintain their independence in the individual's chosen home. Services include advocacy, accessing quality medical & dental professionals, banking and money management, medication management, shopping, home and community safety, meal preparation, cleaning, personal health and hygiene, independent recreation, and help in accessing public transportation. APPLICATION PROCEDURE Call or email for information or to apply. Some clients are referred to the independent living skills program through the Regional Center. Individuals referred to the independent living skills component by one of the State of California Regional Center's must have their Individual Program Plan (IPP) reflect their desire to increase their independence. Individuals with a disease or disability can also self refer. FEES/PAYMENT SOURCE Fees vary.
