Family and Community Medicine ( Low Income )


The agency provides health services at two outpatient clinics in Tarzana and in the Antelope Valley. Services include a community clinic, contract clinic within the LA County contract clinic program, family practice, general medical care, physical examinations, pediatrics, child health and disability prevention (CHDP) exams, and wellness programs.

Contract clinic services include outpatient health services for uninsured, low-income Los Angeles County residents of all ages, including General Relief recipients. It provides primary general medical care. Applicant's household income must be at or below 134% of federal poverty guidelines, or who are GR recipients.

CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.

The program provides smoking cessation for gay, lesbian, bisexual, transgender and HIV smokers. The center is a partner with This is a smoking cessation program consisting of six 2 hour sessions that is sponsored by the L A County Tobacco Control and Prevention Program. The program is facilitated by clinic leaders certified by the American Lung Association. There are no geographic restrictions. APPLICATION PROCEDURE Call to apply, appointments required; walk-ins are accepted. FEES/PAYMENT SOURCE There are fixed fees set by L A County contract clinic program; people who qualify under federal poverty guidelines pay no fees for physical examinations or medication. Individuals qualify if they or their family members are on GR, Medi-Cal, or are uninsured and meet the financial qualifications.
