General Medical Care ( Low Income )

The center provides primary health services for people who live in the Southwest area of Los Angeles County including Artesia, Bellflower, Cerritos, Hawaiian Gardens, Lakewood, Norwalk and Signal Hill. The clinic is an L A County DHS clinic provider. Services include the adult state/local health insurance programs through Medi-Cal managed care and Ability to Pay (ATP); Child Health and Disability Prevention program exams (CHDP); childhood immunizations; general medical care; contraception; prenatal care; pediatrics and well-baby clinics and public clinic. There are no geographic restrictions.

Medi-Cal enrollment and services are restricted to low income people ages 19 to 64, who are L A County Residents, US Citizens, or are legal immigrants and who are not eligible under insurance plans, or other third party coverage. Income must be at or below 133 % of Federal Poverty Level. L A County geographic restrictions apply.

CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.

Contraception and reproductive health services are provided. Low-income individuals may qualify for services under the State Family PACT program. People whose income is less than 200% of the Federal Poverty Level are eligible. Services under Family PACT include a complete physical examination, STD testing and treatment, and contraceptive counseling and services. The Family PACT program serves females from 12 to 55 years, and males from 12 to 65 years. Reproductive health services include colposcopy; case specific pregnancy termination; mammography; and pap smears. APPLICATION PROCEDURE Call to apply. FEES/PAYMENT SOURCE There is a $60 fee per visit for L A County residents; other fees vary, some services are free. CHDP, CHP, Family PACT, Ability to Pay (ATP), Medi-Cal managed care plans are accepted.
