In Home Assistance ( Older Adults )
Provider: BEACH CITIES HEALTH DISTRICT COMMUNITY SERVICES DEPARTMENT
The agency provides services for older adults age 60 and older or disabled adults age 18 and older who live in Hermosa Beach, Manhattan Beach and Redondo Beach. Services include adult respite care; aging/older adult support groups; caregiver/care receiver support groups; case/care management; friendly visiting; in home assistance through the Senior & Disabled Health Fund; peer counseling; walking programs; and wellness programs. Services are restricted to residents of Hermosa Beach, Manhattan Beach and Redondo Beach.
Care management staff conduct in-home visits with older adults and disabled adults, develop individualized care plans, and arrange for needed services. The care manager continues to monitor and evaluate the participant adult to ensure that appropriate services are provided.
The Senior & Disabled Health Fund provides limited funding for in-home assistance for income and need qualified disabled adults and older adults. Assistance may include personal care, light housekeeping, respite for a family caregiver, and emergency response.
A wide range of volunteer programs provide support through social visitors, errand volunteers, telephone support calls and in-home exercise. Volunteers are trained and supervised by professional staff. APPLICATION PROCEDURE Call to apply for service. FEES/PAYMENT SOURCE There are no fees for most services. Fixed fees may be charged for some services; call for details and eligibility.
