Provider: SAN FERNANDO VALLEY INTERFAITH COUNCIL - ALICIA BROADOUS-DUNCAN MULTIPURPOSE CENTER

The program provides a variety of services for adults 60 years and older who live in the northeast San Fernando Valley. Services include blood pressure screenings, case/care management, congregate meals/ nutrition sites, English as a Second Language, friendly visiting, home delivered meals, in-home assistance, legal counseling, Medicare information/counseling, peer counseling, physical fitness, podiatry/foot care, recreational activities/sports, senior centers, senior ride programs, specialized information and referral, tax information, volunteer opportunities, and welfare rights assistance. All services are restricted to residents of Arleta, Lake View Terrace, Mission Hills, Pacoima, Sunland, Sylmar, Tujunga and part of Sun Valley. There are geographic restrictions.

Congregate meal services are provided at all agency locations. Lunch is served Monday through Friday from 11:30am to 12:00pm.

The program provides adult day care for ambulatory seniors. The program includes supervised care, meals, recreational activities and a support group for caregivers.

The case management program serves frail, homebound seniors, 60 years of age and older. The goal of the program is to prevent premature placement of a senior in any type of institution, and to permit them to live independently in their own home. The program is based on a comprehensive assessment of the individual's needs, usually done in the client's home. A plan is then developed to help link the senior with the appropriate services and resources. Monitoring and follow-up is provided to assure the needs are being met

The program provides advocacy for seniors to help them obtain benefits to which they are entitled. This includes assistance with Medicare, Social Security and SSI.

The program provides a wide range of services and assistance to older adults, including arts and crafts, exercise and fitness classes, friendly visiting, health screening, legal services, peer counseling, self-improvement classes, specialized information and referral, tax assistance, a travel club and volunteer opportunities. Classes include English as a Second Language (ESL). Health screenings include blood pressure checks and visits by a podiatrist. A mobile health screening vehicle visits the center on a regular basis. The center provides transportation to and from the center and to medical and other appointments within the service area. Tax assistance is usually available from January through April of each year.

In-home supportive services are provided primarily to seniors who receive SSI and have a limited ability to leave their homes.

The Traditional Legal Assistance Program (TLAP) provides legal assistance including general counseling, benefits assistance, certificates/forms assistance, naturalization assistance, and tax information provided by visiting attorneys from Bet Tzedek. APPLICATION PROCEDURE Call or walk in for information. Call a minimum of three days in advance for transportation. Appointments are required for legal counseling and podiatry services. Reservations are required for the congregate meal. FEES/PAYMENT SOURCE There are no fees for most services. There is a suggested donation of $2.50 for congregate meals for seniors. Younger guests must contribute $4.50.
