Provider: LIFE STEPS FOUNDATION - LANCASTER OFFICE

The program provides a variety of in-home support and training services for frail elderly and for adults who have special needs in the Los Angeles County area.

Those eligible for the in-home services include people who have developmental disabilities, those who are physically disabled, the chronically ill and the elderly. Services offered mobility training, supportive living training, independent living skills training, self-advocacy training, managed care for the elderly, clinical counseling for the chronically ill, and community social integration. All services are available in the home of the client and at the centers. APPLICATION PROCEDURE Referrals from the Regional Center, community agencies, doctors or self-referrals are required. FEES/PAYMENT SOURCE Fees are on a sliding scale for in-home services. The agency also accepts payment through HMO's, PPO's and other medical insurances. Funds are also provided through federal and state grants/contracts such as Regional Center.
