The program provides end of life services for terminally ill adults in the San Fernando Valley, San Gabriel Valley and parts of the Greater Los Angeles area. Service includes in-home hospice care. A diagnosis of terminal illness with a six-month life expectancy must be verified by the patient's attending physician. Some geographic restrictions apply.

The hospice program provides the services of a multi-disciplinary team which includes bereavement/spiritual counselors, certified home health aides, clergy, medical social workers, pharmacists, physicians, physical, occupational and speech therapists, registered nurses, and volunteers. Services include in-home care coordinated with the patient's physician; in-home health aides; nursing visits; medical equipment, supplies and specific medications; spiritual care by hospice counselors or local clergy; support for family and loved ones; respite care; hospital inpatient care for symptom control; physical, occupational and speech therapies; 14-month bereavement counseling program for family/caregiver following the patient's death. APPLICATION PROCEDURE Call for information or to apply for service. FEES/PAYMENT SOURCE There are fixed fees. The program accepts Medicare, Medi-Cal and private insurance. A sliding scale is available for individuals who do not have insurance.
