Hospice Care


The agency provides end of life services for terminally ill patients and their families in the San Gabriel Valley. Services include bereavement support groups, disease/disability information, hospice care, terminal illness counseling, and volunteer opportunities. Geographic restrictions apply for services.

Most services are provided in the person's home; short-term respite care and symptom control are provided in the agency's homelike ten bed inpatient facility or in contracted skilled nursing facilities.

Services assist terminally ill people to live as fully and comfortably as possible while recognizing death as a normal process, by providing support and care that neither postpones nor hastens dying.

The program combines the services of interdisciplinary caregivers and involves the patient and family in care decisions whenever possible. In addition to providing direct services staff can offer case management assistance by locating and coordinating other services the person or family may need. Respite care enables family members to have a short break from their full-time care responsibilities.

The program provides speakers for community groups who can present information about hospice care and encourage community involvement.

The program recruits, trains, and utilizes adult volunteers who assist in a variety of areas including patient and family care. APPLICATION PROCEDURE Call to apply. A prognosis of six months or less is required for hospice care. Care is provided under the direction of the person's personal physician. FEES/PAYMENT SOURCE The agency accepts what Medicare, Medi-Cal and private insurance reimburse as payment in full. Limited resources for care are available for people without insurance.
