PACE is a managed care program that provides medical, social, nutritional and rehabilitative services to older adults. The program allows older adults who are frail and meet nursing home eligibility criteria to live in their own homes. The program includes primary medical and specialty care, prescription medications, basic dental care, adult day health care (ADHC) services, and hospitalization as needed. The central intake number offers an option to receive information specific to seniors and older adult caregivers. Staff can assess and route calls for Medicare or health information, or make an appointment for primary care service at one of the agency's clinic locations. All languages are served through an interpretation service APPLICATION PROCEDURE Call or walk in to apply. FEES/PAYMENT SOURCE There are fixed fees for services; Medi-Cal, Medicare and private insurance are accepted.
