Long Term Case/Care Management ( AIDS )
Provider: FOOTHILL AIDS PROJECT
The program provides HIV/AIDS services for people with HIV/AIDS who live in the eastern part of the San Gabriel Valley in Los Angeles County, San Bernardino County, and western and central Riverside County. Services include case management, health education, HIV testing, health/disability related counseling and mutual support groups, and sex offender counseling. There are no geographic restrictions.
Case management services include housing location assistance, mental health counseling and support groups, emergency food, hotel/motel vouchers, substance abuse treatment, taxi vouchers and bus passes.
Legal services, medical services, public benefits advocacy and other services are available through referrals to local HIV/AIDS service providers.
The program also provides HIV/AIDS prevention and education through presentations to schools, colleges, community groups and agencies. APPLICATION PROCEDURE Call or walk in for information or to apply. Proof of diagnosis, residence and income are required. Health insurance verification is done at intake; however, health insurance is not required for service.
