Medicare Information/Counseling ( Asian Community )
The agency is authorized to conduct enrollment into Covered California, Medi-Cal, and other public health insurance programs for individuals and families, who are without health insurance and are not covered by any other social or private insurance programs. Services include state/local health insurance information/counseling, medicaid applications, medicare information/counseling, certificate forms assistance for food stamps/SNAP applications and WIC application/certification. There are geographic restrictions for some services. In addition, the agency provides Medi-Cal counseling for low-income adults and Medicare Part D counseling for older adults. The outreach programs target the Asian/Pacific Islander community, and offer translation and transportation to encourage understanding and enrollment.
The agency also provides information and assists with applications for food stamps/SNAP and WIC. APPLICATION PROCEDURE Call the agency's insurance enrollment hotline number to find out about enrollment in Healthy Kids or other free/low-cost insurance programs, and to make an appointment. Callers who leave a message will be called back by an enrollment agent to schedule an appointment.
Documentation requires proof of family income, including previous year's income tax form; proof of residence, such as a utility bill, rent receipt or driver's license. The person applying may be the parent, step parent, guardian or relative care giver. There are no fees to enroll; premiums for select programs are based on the plan selected and other program criteria.
