The clinic provides health services for low income people of all ages in the San Fernando Valley. Services include child health and disability prevention (CHDP) exams, childhood immunizations, community clinics, contract clinics, dental care, general health education programs, lead testing, optometry, pediatrics, prenatal care, sexually transmitted disease screening, sexually transmitted disease treatment and vision screening. Optometry services are limited to those who live in the San Fernando Valley, but not to those who live in the City of San Fernando. There are no geographic restrictions for other services.

Contract clinic services include outpatient health services for uninsured, low-income Los Angeles County residents age 19 to 64, including General Relief recipients. It provides primary general medical care. Applicant's household income must be at or below 134% of federal poverty guidelines, or who are GR recipients. People who have AIDS/HIV and who qualify for Healthy Way LA may receive primary care at the agency's North Hollywood location.

CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.

Lead level testing is available for children newborn to 18 years.

Optometry services include vision screening, glaucoma testing and corrective lenses.

Due to the overwhelming demand for dental services, the agency is only able to provide dental treatment for established Clinic patients. APPLICATION PROCEDURE Call to apply; an appointment is usually required. An appointment for any service can generally be scheduled within a week of the request. FEES/PAYMENT SOURCE Most services are set on a sliding fee scale starting at $0; Medi-Cal is accepted. Fees for vision exams vary and start at $40, fees for eyeglasses vary and start at $50. There is no fee for educational presentations in the community. Eligible contract clinic patients pay no fees.
