Provider: CALIFORNIA STATE UNIVERSITY NORTHRIDGE - DEPARTMENT OF SOCIAL AND BEHAVIORAL SCIENCES - ASSESSMENT CLINIC

The agency provides services for people with disabilities, age 2 through 90. Services include audiological evaluations, developmental assessment, psychological assessment, and special education assessment. There are no geographic restrictions.

The assessment clinic accepts referrals concerning evaluation of developmental, emotional and behavioral issues; as well as difficulty with reading (dyslexia), written language (dysgraphia), and math (dyscalculia), and ADD (attention deficit disorder) or AD/HD (attention deficit/hyperactive disorder) in children, adolescents and adults.

Referrals are received from professionals, parents, guardians or teachers. The person referring a child must complete a questionnaire and objective rating scales regarding the child's general health, medical history and development. Comprehensive assessments usually take from one full day to several half days. APPLICATION PROCEDURE Assessments are by appointment only. Call the clinic director, Dr. Katz (pronounced Kates), to make an appointment. FEES/PAYMENT SOURCE Fees for evaluations are based on the length and complexity of the assessment, and typically range from $250 to $350.
