State/Local Health Insurance Information/Counseling ( Youth )

The agency is authorized to conduct enrollment in the Healthy Kids and Medi-Cal insurance programs and Kaiser (when available) for families who have children, age zero to five, who are without health insurance and are not covered by any other social or private insurance programs. Services include state/local health insurance information/counseling. The insurance program is provided by the Children's Health Initiative of Greater Los Angeles, and LA Care Health Plan (see the separate entries for details). The agency assists parents in completing the applications, which are forwarded to the insurers. The agency serves the cities of Altadena, Pasadena and Sierra Madre. APPLICATION PROCEDURE Call the agency's number for health insurance information to find out about enrollment in the insurance programs and for an appointment.

Documentation required may include proof of family income, including previous year's income tax form; proof of residence, such as a utility bill, rent receipt or driver's license and other documentation may be required depending on the health insurance program. The person applying may be the parent, step parent, guardian or relative care giver. FEES/PAYMENT SOURCE There are no fees to enroll; premiums are based on the number of children enrolled.
