Transitional Case/Care Management ( Homeless Families, Parenting Issues )

The FAMILY REUNIFICATION HOUSING SUBSIDY (FRHS) program provides housing services to homeless participants identified by the Children's Dependency Court, so they can be reunified with their children who have been under the custody of the child welfare system. There are no geographic restrictions.

The BRINGING FAMILIES HOME (BFH) program provides housing services to eligible homeless families who are currently receiving court ordered or voluntary family maintenance services, so the parents can maintain custody of the children in their home. APPLICATION PROCEDURE For the FRHS program homeless parents must obtain a court order for housing assessment, signed by a judicial officer and contact the 211 FRHS office on the 3rd floor of the Children's Court, Rm. 3005.

For the BFH program homeless families must be referred by their social worker and contact the 211 BFH office on the 3rd floor of the Children's Court, Rm. 3005. FEES/PAYMENT SOURCE There are no fees for services.
