Hospice Care Referrals

The foundation provides end of life services for terminally ill people and their families throughout California and Nevada. Services include hospice referrals, professional continuing education and public awareness/education. The purpose of the foundation is to provide support for local hospices to enable them to offer high quality care, to raise public awareness of the hospice care option, and to offer education for professionals. There are no geographic restrictions.

Information and education for the public is provided through printed materials and conferences, and continuing education for professionals through seminars and workshops, and networking with hospice-related agencies. The agency provides information about hospice programs in California; referrals are made to member hospices; a referral does not indicate a recommendation or a standard of care.
