Care Coordinator Specialist (Remote)

NOTE: The position is conducted remotely during COVID-19, but may resume on-site services once it is safe to do so.

Responsible for providing essential care coordination services to clients referred to select special programs.  Assess and refer clients to resources and support providers.  Identify any appropriate human service programs and community resources using CareLinQ and provide follow-up services in accordance with the program.  Communicate with partner agencies in an effort to answer any questions regarding 211 services and resources to meet the needs of clients.  Responsible for tracking various project outcomes. The Care Coordinator will also participate in community town halls, discussions, and educational and outreach opportunities to help gather information about community and family needs and educate community members about the program.

Program Assistant

As a 211 LA County Program Assistant you will be responsible for supporting the Program Manager or Director by providing administrative and project support. 211 LA is a fast paced work environment in which it is necessary to provide support in a performance driven, 24/7 social service operation.

Development Director

The Development Director will lead the design and implementation of new programs, develop fundraising strategies, and contribute to the overall strategic growth of the organization. Responsibilities include project planning and management, creating protocols and workflows, invoicing and contract management, and facilitating intra-agency coordination to successfully launch new programs.
