Congregate Meals/Nutrition Sites

The program provides older adult services to low-income functionally impaired adults age 18 and older and older adults age 60 and older who live in La Mirada, Pico Rivera, Santa Fe Springs and Whittier. Services include congregate meals, case/care management, home delivered meals, telephone reassurance, and respite care. Services are restricted to residents of La Mirada, Los Nietos, Pico Rivera, Santa Fe Springs, Whittier and surrounding areas.
The agency provides congregate meals and home delivered meals. Both nutrition programs for older adults age 60 and older who live in the southern part of the San Gabriel Valley and the southeast area of Los Angeles County. Both programs provide a hot lunch; the congregate meal program also provides an opportunity to socialize and participate in coordinated activities.
Telephone reassurance is provided on request.
Respite care services include referrals for additional services including support groups and counseling, and access to a caregiver resource library.
The Supportive Services Program (SSP) and Linkages program provide case management services for older adults age 60 and older and persons with disabilities to allow them to stay in their homes as long as appropriately possible and prevent premature or inappropriate institutionalization. Case/care management services include an initial comprehensive assessment, coordination of all required services, and follow-up care. Referrals for additional services are provided as necessary.
