The agency provides case management for veterans in Los Angeles County. Services include case/care management for previously homeless veterans through the Aftercare Program There are no geographic restrictions.

Aftercare is a specialized program to support veterans who have secured permanent housing to maintain stability and prevent relapse into homelessness, including those who are currently working with SSVF and HUD/VASH programs. Veterans must be 18 years or older, have a history of homelessness (from one night to several years), and currently reside in permanent housing (with lease or agreement, and mailing address).  

Services include financial management assistance, meal planning assistance, career assistance referrals, health and mental health service referrals, veteran benefits assistance referrals, and tenant rights and responsibilities information.
