Adult In Home Respite Care

Provider: HUNTINGTON HOSPITAL SENIOR CARE NETWORK

The program provides services for frail older adults 65 years and older who live in the San Gabriel Valley and Pomona Valley. Services include care management and health and supportive services for low income people who are at high risk for institutionalization without these services. To qualify, people must be certified or certifiable for a skilled nursing level of care; must be eligible for Medi-Cal without a share-of-cost provision. People who are currently in residential care facilities and nursing homes who could live independently in the community with supportive services are also eligible. Geographic restrictions apply.

   The service area is restricted to the cities of Alhambra, Altadena, Arcadia, Azusa, Baldwin Park, Bassett, Burbank, Claremont, Covina, Diamond Bar, Duarte/Bradbury, El Monte, Glendale, Glendora, Hacienda Heights, Industry, Irwindale, La Canada/Flintridge, La Crescenta, La Habra Heights, La Puente, La Verne, Monrovia, Monterey Park, Montrose, Pasadena, Pomona, Rosemead, Rowland Heights, San Dimas, San Gabriel, San Marino, Sierra Madre, South El Monte, South Pasadena, South San Gabriel, Temple City, Walnut and West Covina.

   Referrals for counseling, medical care, financial assistance, home health care, homemakers, home repair, estate planning, legal assistance, transportation and escort services, adult day and respite care, friendly visiting, personal care, home-delivered meals, residential options, and other resources are available. Staff are available to meet with the client and significant others in the home; to develop a care plan for needed in-home services; and to arrange for, coordinate, and monitor the services. Staff also assist families who want to arrange to relocate a senior to a retirement or nursing home; or want resources for relatives who live elsewhere.

    The Multipurpose Senior Services Program (MSSP) provides ongoing care management services for low-income, Medi-Cal approved, frail elderly people to enable them to continue to live at home; it includes financial assistance for the purchase of needed services.

     Assisted Living Waiver (ALW) is a program that seeks to move Medi-Cal recipients out of long-term skilled nursing placements into a lower level of care while still providing for their needs, including care coordination, medication assistance and other services. Residential care facilities that are capable of providing this type of care are enrolled in the program. Huntington Senior Care Network provides monthly monitoring to assure that clients are well-served.

  Educational programs include lecture/discussion meetings about health issues like vision and hearing, medication, Alzheimer's disease, heart disease and exercise. The educational programs are also available for family members and caregivers. Speakers are available for community groups.

  Adults age 50 and older may become members of the 50 (plus) Health Connection to attend free seminars about health and wellness topics, and receive a newsletter which contains information related to health, wellness and senior services. The program also provides discounts on hearing aids, glasses, prescriptions, meals at the hospital cafeteria, items at the hospital gift shop and other community goods and services.

  Staff also provide support for the family and other caregivers of the client. Services include education and skill building classes, access to resources such as individual counseling, support groups and an out-of-area referral network. The types of services provided are contingent on client need. Services may continue as long as the client is eligible for and wants to remain in the program.
