Housing Search Assistance
Provider: ST. JOSEPH CENTER
The Senior Service Program provides information and referral services for low income seniors age 60 and older. It also provides in-home assessment, care coordination, and a supplemental grocery program for seniors who meet the same criteria and are unable to leave their homes. The program is designed to link SSI income-level seniors with the community resources they need and to prevent inappropriate institutionalization. Homeless older adults are provided with case management, referrals, and housing placement.
Seniors who are unable to leave their homes may call to arrange for an in-home needs assessment. Staff work with the seniors to identify needs and services that can meet them, link the senior with services and advocate if necessary, and provide short or long-term case coordination when appropriate.
Seniors who are unable to leave their homes may also apply for the supplemental food program. Participants receive a bag of groceries weekly which contains canned and dried items, and may also receive case coordination or other services from the program, as needed.
