Transitional Case/Care Management

The agency provides services for older adults over the age of 65 who live in Los Angeles, Orange, Riverside, San Bernardino, San Diego and Ventura Counties. Services include specialized information and referral and transitional case/care management. There are no geographic restrictions.

Staff assess the senior's needs and provides referrals to the appropriate resources.

Information and referral may be on a variety of senior topics and or problems in English and Spanish, such as but not limited to assisted living referrals; in-home care; wheelchair maintenance and cleaning; free cell phone for low income seniors; financial assistance program information; pharmacy/prescription home delivery program; palliative or end of life care; food program referrals (local and home delivery); low-income dental programs; skilled nursing facility placement; transportation assistance services and veteran benefits assistance (financial assistance, housing assistance or home modification).

Case/care management is provided with in-home visits for services, such as a fall prevention/mobility program; home based medical services; medication management (checking for harmful drug interactions); Medi-Cal application assistance; mobile doctor/house call doctors; and mobility equipment evaluations.
