OIG Hotline Operations is part of a Federal Law Enforcement Agency and accepts reports and complaints about fraud, misconduct, mismanagement and waste in all HHS-administered programs. HHS regulates the effectiveness of Medicare, Medicaid, public health, medical research, food and drug safety, welfare, child and family services, disease prevention, Indian Health, and mental health services. There are no geographic restrictions.

People may call the hotline to report crime or fraud by employees or contractors of the following HHS Divisions: Administration for Children and Families (ACF), Child Support Enforcement (CSE), Centers for Medicare and Medicaid Services (CMS), Food and Drug Administration (FDA), National Institute of Health (NIH), Office of Disease Control and Prevention (CDC), Indian Health Service (IHS), Office of Inspector General (OIG), Office of the Secretary (OS), Health Resources and Services Administration (HRSA), Substance Abuse and Mental Health Administration (SAMSHA), Administration on Aging (AOA) Agency for Health Care Policy and Research.

Once a complaint has been received, an OIG analyst will review the complaint for relevance and completeness. Not all complaints result in an investigation. If you have identified yourself, a reviewing official may contact you for further information. OIG Hotline Operations is not authorized to disclose any information on records in its possession. OIG Hotline Operations will not confirm receipt of a complaint or respond to any inquiries about action taken on a complaint.

Application Procedure
Complaints can be filed online at https://oig.hhs.gov/fraud/report-fraud People who call the hotline and those who prefer to mail or FAX their complaints or reports of suspected fraud are asked to provide their name, address, phone, and email information. If they wish to report in confidence, they may omit this personal information. The report must also include the name and title of the person alleged to have committed the fraud; the address, phone, and email; and a short explanation of the circumstances of the allegation.
