Case/Care Management

The center provides services for older adults age 55 and older who live in the West Wilshire area of Los Angeles. Services include case/care management. Age and income restrictions apply for some services; services are restricted to residents of the Hollywood-Wilshire area.

The Homeless Prevention Program provides case management with mental health therapy for older adults in SPA's 4 and 5, who are living with a hoarding disorder and are at imminent risk of eviction. A mental health team will assist with declutter/cleanout services, harm reduction approaches, resolution of eviction issues and counseling. A clinician or care navigator will provide weekly in-home visits.
