What is the Enhanced Care Management (ECM) Program?

The Enhanced Care Management (ECM) Program delivers holistic support and coordinated services to high-need Medi-Cal beneficiaries. Enhanced Care Management (ECM) is a Medi-Cal benefit that provides comprehensive care management services to Medi-Cal members with complex health and/or social needs who meet the eligibility criteria, part of the DHCS CalAIM initiative.

How to get ECM services

Our Program is designed for Medi-Cal members in Los Angeles County who may qualify for Enhanced Care Management services. To receive ECM Services, you must:

  • Be enrolled and receiving care through a managed care plan (e.g, Health Net, Kaiser, or Molina Healthcare)
  • Meet certain qualification terms.

Program Eligibility Requirements

To participate in the Enhanced Care Management Program (ECM), applicants must meet certain eligibility requirements, such as: 

  • Adults & Children Experiencing Homelessness & Adults with Families Experiencing Homelessness
  • Adults & Children at risk of ED Utilization or Hospitalization
  • Adults & Children with Serious Mental Illness/Substance Use Disorder (SMI/SUD) and other health needs
  • Adults & Children at Risk of Long Term Care or Adults Transitioning into the Community from Long Term Care

If you are not eligible for this program, 211 LA is committed to assisting you via our direct line that operates 24 hours, 7 days a week.
