Child Health and Disability Prevention Exams
Provider: CHILDREN'S CLINIC, THE - FAMILY HEALTH CENTER IN NORTH LONG BEACH AT HAMILTON MIDDLE SCHOOL
The clinic provides comprehensive outpatient health services for people who live in Long Beach and surrounding areas. Services are targeted to low-income families who have little or no medical insurance. Services are provided from nine sites. There are no geographic restrictions.
Services include well-child exams, general and specialty pediatric services; immunizations and TB testing; Child Health and Disability Prevention (CHDP) exams; prescribed medications; laboratory testing; acute and chronic care for children and adults; community outreach and education; an adolescent clinic and an adult clinic; and referrals to community resources. The clinics also provide primary general medical care for homeless people and participate in the L A County contract clinic program.
Contract clinic services include outpatient health services for uninsured, low-income Los Angeles County residents of all ages, including General Relief recipients. It provides primary general medical care.
The clinic at the Children's Clinic Family Health Center location also provides pediatric dentistry in the contract clinic program. Applicant's household income must be at or below 134% of federal poverty guidelines, or who are GR recipients.
CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.
