Specialized Information and Referral

Provider: USC FAMILY CAREGIVER SUPPORT CENTER

The center, in partnership with AARP and 211 LA County, provides services for caregivers caring for adults age 55 and older with a chronic health condition or of any age with a brain-impairing condition diagnosed in adulthood (18). Services include caregiver/care receiver support groups and specialized information and referral for caregivers. Services are targeted to, but not restricted to the hispanic/latino community. There are no geographic restrictions.

The program Ayudando a Quien Ayuda, also known as Helping the Helper, matches caregivers with a resource navigator who will create a personalized plan to provide supportive services that may include additional information, assessment, individual consultations, respite, education, and training.

To be eligible for the program, caregivers must be providing care for a family member or close friend who is age 55 and older, or is of any age if living with a chronic brain injury that developed after the age of 18. This program determines a caregiver is someone who spends several hours a week or more helping with:

- daily activities including dressing, bathing, preparing meals, or cleaning
- running medical and personal errands
- assisting with medication management and other medical needs
