The center is a community clinic that provides health services for people of all ages who live, primarily, in the East San Gabriel Valley. Services include a general family practice which provides diagnosis, treatment, lab tests, EKG's, blood pressure checks, diabetes management, dental care, physical exams, and routine medical care for people of all ages; pediatric services which include well baby clinics, free immunizations, school and sports physicals, and CHDP (Child Health and Disability Prevention) Exams; annual exams, nutrition education, treatment of minor infections; tuberculosis screening; sexually transmitted diseases for both men and women; and limited pharmacy services. There are no geographic restrictions.
Contract clinic services include outpatient health services for uninsured, low-income Los Angeles County residents of all ages, including General Relief recipients. It provides primary general medical care. Applicant's household income must be at or below 134% of federal poverty guidelines, or who are GR recipients.
CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.
Dental services include examinations, cleanings, x-rays, fillings, fluoride treatments, crowns and bridges and dental emergencies.
The clinic provides a Comprehensive Perinatal Services Program (CPSP) for women who are eligible for Medi-Cal. The program provides nutrition and health education, psychosocial services and basic prenatal and obstetrical care.
