Case/Care Management (Frail Elderly)

Provider: WILMINGTON JAYCEES FOUNDATION INC

The program provides services for older adults 60 years and older who live, primarily in the Harbor City, Harbor Gateway, San Pedro, Wilmington area. Services include an in-home assistance registry, case/care management services, congregate meals and home delivered meals. Transportation for congregate meals may be available. Age and geographic restrictions apply; home delivered meals are restricted to residents of zip codes 90501, 90710, 90731, 90732, and 90744.

The program provides an in-home assessment for elderly clients. Case managers assist clients to find appropriate care. There is also an in-home assistance registry of aides who can provide homemaking or personal care in the client's home, if necessary.

Home delivered meals are available for homebound and frail elderly individuals. Hot meals are delivered five days per week.
