Services include case/care management; general medical care, including chronic disease management services; general physical exams; mental health evaluation; psychiatric medication monitoring; dental care; family planning; and optometry. There are no geographic restrictions.

Ancilliary services include radiology, lab services and pharmacy services. Staff will assist with medical insurance eligibility.

Fees

Fees vary; services are set on a sliding scale for individuals at or below 200% of Federal Poverty Income Guidelines. Medi-Cal, Medicare and HMO insurance are accepted. No one is turned away for lack of ability to pay or due to being uninsured.
