The program provides health services for people of all ages and from any area comprehensive medical diagnostic, treatment, rehabilitation, and consultation services. The program provides services for patients with disorders of the brain, spinal cord, nerves and muscles. The services are provided through a variety of coordinated, specialized medical clinics including a neurorehabilitation program. There are no geographic restrictions.

Application Procedure
Call for information or to arrange a medical appointment through the number listed above. Patients who are in managed care health systems should obtain documented authorization prior to calling for patient appointments. People may also be referred for service by a private physician or medical facility. First time patients call to pre-register as a new patient.
Fees
There are no fees for information or referral. The fee for medical services varies with the type of care needed. Medicare and private insurance are accepted. The program welcomes donations to offset the cost of services provided and to support ongoing research.
