Psychiatric Case Management
Provider: LOS ANGELES COUNTY DEPARTMENT OF MENTAL HEALTH - COMPTON FAMILY MENTAL HEALTH CENTER
The agency provides outpatient mental health services, diagnosis, treatment, crisis intervention, and case management services for adults age 18 and older who are in crisis and/or who are acutely and severely or chronically mentally ill who live in Compton, Lynwood, Paramount, and Willowbrook. The target population for the agency are people who are acutely and severely and/or chronically mentally ill; however, anyone who needs assistance may apply and the agency can refer, if necessary, to other resources.
Services include evaluation for voluntary or involuntary hospitalization and medication; short-term group and individual treatment; medication supervision; and on-site crisis intervention during specific hours. Services also include case management for clients who receive SSI, counseling, case management, and supportive services for adults who have been discharged from state, county, and county-contract psychiatric hospitals to help them make the transition back to the community and prevent re-hospitalization.
Mental health evaluation is done with clients who meet the eligibility requirements and are seeking ongoing treatment at the center. The center does not provide court-ordered mental health evaluation.
The agency also provides an MHSA funded Wellness Center to provide services and support to mental health clients who are in recovery and no longer need intensive mental health services. The Wellness Center provides self-help mental health support groups; peer to peer support groups; social/recreational activities; medication evaluation and support services; employment and education services, and integrated services and support for co-occurring disorders, such as substance abuse.
In addition the centers provide physical health screenings, and assistance with referrals to health care providers. The program also provides healthy living and health management education. Referrals are given for, housing possibilities, benefit maintenance and short term or crisis mental health services.
The center also provides a full day rehabilitation program of social, vocational and psychological rehabilitative services.
