Disease Prevention Programs (Older Adults)

The agency provides health and fitness services for older adults age 60 and older in Los Angeles county. Services include chronic disease self management programs, disease prevention programs, fall prevention programs, physical fitness, and a home barrier evaluation/removal for older adults. Age restrictions apply; there are no geographic restrictions.
The Disease Prevention and Health Promotion program (DPHP) is an evidence based program focused on preventing illness and managing chronic physical conditions. It teaches healthy living habits including nutritious eating and relaxation techniques for conditions such as arthritis, heart disease, high blood pressure, lung disease, and cancer; chronic pain, and diabetes. Workshops are available online, in-person and by telephone.
The Dignity at Home Fall Prevention Program (DHFPP) is available for older adults who have fallen in the past, have a risk of falling, or have a fear of falling. Programs include A Matter of Balance, Bingocize, Tai Chi For Arthritis, Healthy Moves for Aging Well, Enhanced Fitness, and an Arthritis Foundation exercise program. Sessions are avaialble online or in-person.
The Older Adults Home Modification Program (OAHMP) enables low-income older adults to remain in their homes with low-cost modifications including grab bars, handrails, doorknobs, faucets, handheld shower heads, tub cuts, non-slip strips, improved lighting, temporary ramps or stair railings. Applicants must be 62 years or older, own or rent their primary residence, and meet income eligibility guidelines set by HUD.
