Transitional Case/Care Management
Provider: SOUTHERN CALIFORNIA HEALTH AND REHABILITATION PROGRAM
The agency provides youth services for young adults ages 18 to 24 in Los Angeles county. Services include transitional case/care management for transitional age youth with a serious emotional disturbance or persistent mental illness. Age restrictions apply; there are no geographic restrictions.
The program assists qualified youth who are homeless or at risk of becoming homeless, aging out of the mental health or juvenile justice system, exiting long term residential care, or are otherwise unable to access traditional mental health care. Care includes an individual service plan; mental health services, which can be individual or group counseling; assistance with housing or CES; and linkage to community resources. Youth may be referred by DCFS; social service agencies or may self-refer.
