Group Counseling (Chronic/Severe Mental Illness)
Provider: ST. ANNE'S RESIDENTIAL FACILITY
St. Anne's Mental Health Program provides mental health services for people with chronic/severe mental health issues, who are full scope Medi-Cal recipients. Services include individual counseling, family counseling, play therapy, group therapy and wraparound services. Ancillary services include therapeutic rehabilitation, medication support, crisis intervention, case management. The program serves people who live, primarily, in the metro area of Los Angeles, SPA 4; however, there are no geographic restrictions.
The program serves both the young women and their children residing at the agency and individuals birth to twenty-one years of age living in the community. Each client must meet the Department of Mental Health medical necessity criteria and be full-scope Medi-Cal eligible. Services are offered to the client and their family at St. Anne's and community settings such as the client's home or school. Each client receives a full mental health assessment, individual therapeutic interventions a minimum of once a week, targeted case management, and medication support services as needed.
Wraparound is conducted by a multi-displinary team consisting of a social worker case manager, a therapist and a parent "Partner". Sessions may involve just the child or the whole family as needed. The program accepts youth from age 8 to 18, and may continue for one year or longer. The service area is L A County SPA 4 (LA Metro area).

Application Procedure
Call to make an appointment. Clients must have full scope Medi-Cal. Required documentation includes copies of social security card, Medi-Cal card, government issued identification card or drivers license, proof of income, proof of address, and immunization record.
