Adult Day Health Programs
Provider: CARSON ADULT DAY HEALTH CARE CENTER
The center provides adult day health care for impaired adults age 18 and older and seniors who live in Carson and the surrounding communities whose physical, mental or social needs make independent living difficult and who may be at risk for institutionalization without supportive care. There are geographic restrictions.
The program provides medical and nursing services; physical, occupational and speech therapy; psychiatric services; nutrition services; recreational and social activities; and breakfast, lunch and snacks. Transportation to and from the program may be provided for individuals who live within the primary service area.

Application Procedure
Call, write or walk in to apply. Picture ID, proof of age, proof of current residence are required; a drivers license or identification card are accepted. Medi-Cal participants must provide their Medi-Cal/social security number.
Fees
There are fixed fees; call for information. Medi-Cal with managed care and private insurance are accepted.
