Provider: KOREAN HEALTH, EDUCATION, INFORMATION AND RESEARCH CENTER - ADMINISTRATION

The agency provides services for older adults in the City of Los Angeles. Services include case/care management and geriatric assessment. Services are targeted, but not restricted, to the Korean community in the Koreatown area. There are no geographic restrictions.

Services includes referral, placement assistance, and follow-up for needed community services.

The program also provides a clinic for older adults that provides a geriatric assessment.

The Supportive Services Program (SSP) provides case management services to assess the physical, psychological, and social needs of older adults age 60 and older to allow more frail seniors to stay in their homes as long as appropriately possible.
