Veteran Homes
Provider: CALIFORNIA STATE DEPARTMENT OF VETERANS AFFAIRS - VETERANS HOMES OF CALIFORNIA - YOUNTVILLE
The program provides out-of-home-care for veterans age 55 and older, as well as disabled veterans and homeless veterans in need of long term care, who are current residents of the State of California. Services include veterans homes. There are no geographic restrictions; however, individuals must be a current resident of California.
The program provides a range of levels of care that include independent living, assisted living, skilled nursing care, and memory care. Ambulatory Care Clinics serve residents of the independent living locations. These clinics provide comprehensive multidisciplinary assessments, well as ongoing primary care.
Properties are located in Chula Vista, Fresno, Lancaster, Ventura, West Los Angeles, Redding and Yountville.
Applicants must meet a range of admission requirements and provide eligibility documentation. There is a waiting list for most locations.
