Case/Care Management
Provider: HARBOR REGIONAL CENTER
The center provides services for people with developmental disabilities of all ages and their families who live in the Bellflower, Harbor, Long Beach and Torrance Health Districts of Los Angeles County. Individuals must have a disability due to autism spectrum disorder, cerebral palsy, epilepsy, intellectual disability or other similar condition requiring the same treatment, which began before their 18th birthday and is expected to continue indefinitely. Geographic and other restrictions apply.
Clients who are found to have a lifelong developmental disability are eligible to receive life-long services. At risk infants and toddlers, and individuals who are at risk of having a child with a developmental disability may also qualify for early childhood services. The center does not serve individuals with disabling conditions that are solely physical in nature.
Direct client services include assessment, diagnosis and early intervention; service coordination and specialized information and referral. Early intervention is available for children, age 3 and younger, who are developmentally delayed, or have conditions that present an established risk of developmental disability, and prenatal diagnosis and genetic counseling services to at-risk pregnant women.
Additional services for families of clients include behavior management and intervention training; advocacy and education; support groups; parent education and networking; and information about guardianship, conservatorship, and living options. A Resource and Toy Lending Library, and Assistive Technology Lab are also available.
