Dementia Evaluation (Korean Community)

This is a community clinic that provides health services for low-income residents of the Koreatown area of Los Angeles. Services include blood pressure screenings; breast examinations; breast self-exam instruction; child health and disability prevention exams (CHDP); childhood immunizations; contraception; contract clinics; community clinics; dementia evaluation; diabetes screening; disease/disability information on disability issues, general dentistry, heart disease, sexually transmitted diseases; flu vaccines; optometry; pap tests; and support groups for cancer and families/friends of drug abusers. There are no geographic restrictions.
Contract clinic services include outpatient health services for uninsured, low-income Los Angeles County residents of all ages, including General Relief recipients. It provides primary general medical care. Applicant's household income must be at or below 134% of federal poverty guidelines, or who are GR recipients.
CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.
The clinic provides screening for high blood pressure, diabetes, heart disease, mammography, pap smears and pelvic ultrasound exams. It also provides immunizations and a flu shot clinic. Programs for the community include HIV prevention, breast self-examination, chronic disease, heart disease, prenatal, perinatal and infant programs, second hand smoking prevention, and STD's.
